• Certificate of Compliance Application
• Workforce Report

If you are selected as a bid finalist, please return this application and all necessary attachments to your Olathe contract liaison. Please include state or federal certification of Minority and/or Women’s Business Enterprises or Joint Ventures with Minority and/or Women’s Business Enterprises, if applicable to your business.
